氨茶碱联合罗红霉素治疗支气管扩张患者的效果及其对肺功能的影响 被引量:4

摘要 目的探讨氨茶碱联合罗红霉素对支气管扩张患者的应用效果及其对肺功能的改善情况。方法 104例支气管扩张患者作为研究对象,随机分为观察组和对照组,各52例。两组均给予常规治疗,对照组采用罗红霉素口服治疗,观察组在此基础上加用氨茶碱治疗。观察比较两组患者的治疗效果、肺功能各项指标[包括第1秒用力呼气量(FEV1)、用力肺活量(FVC)、呼气峰流量(PEF)]和呼吸困难评分。结果观察组总有效率为92.31%,高于对照组的76.92%,差异有统计学意义(P<0.05)。治疗前,两组患者FEV1、FVC、PEF比较,差异无统计学意义(P>0.05)。治疗后,两组FEV1、FVC、PEF均高于治疗前,差异有统计学意义(P<0.05);观察组FEV1、FVC、PEF高于对照组,差异有统计学意义(P<0.05)。治疗前,两组患者呼吸困难评分比较,差异无统计学意义(P>0.05)。治疗后,两组呼吸困难评分均低于治疗前,差异有统计学意义(P<0.05);观察组呼吸困难评分低于对照组,差异有统计学意义(P<0.05)。结论氨茶碱与罗红霉素联用治疗支气管扩张能显著改善患者的肺功能,有效控制感染和减轻患者呼吸困难症状,值得临床推广。 Objective To discuss the effect of aminophylline on patients with bronchiectasis and its improvement on pulmonary function. Methods A total of 104 patients with bronchiectasis as study subjects were randomly divided into observation group and control group, with 52 cases in each group. Both groups received conventional therapy. The control group received oral administration of roxithromycin, and the observation group also received aminophylline for treatment. Observation and comparison were made on treatment effect, pulmonaryfunction indexes [including forced expiratory volume in 1 second(FEV1), forced vital capacity(FVC), peak expiratory flow(PEF) and dyspnea score] between the two groups. Results The observation group had higher total effective rate as 92.31% than 76.92% in the control group, and the difference was statistically significant(P〈0.05). Before treatment, there was no statistically significant difference in FEV1, FVC and PEF(P〉0.05). After treatment, both groups had higher FEV1, FVC and PEF than those before treatment, and their difference was statistically significant(P〈0.05). The observation group had higher FEV1, FVC and PEF than the control group, and the difference was statistically significant(P〈0.05). Before treatment, there was no statistically significant difference in dyspnea score(P〉0.05). After treatment, both groups had lower dyspnea score than those before treatment, and the difference was statistically significant(P〈0.05). The observation group had lower dyspnea score than the control group, and the difference was statistically significant(P〈0.05). Conclusion Combination of aminophylline and roxithromycin can significantly improve pulmonary function of patient with bronchiectasis, effectively control infection and reduce the symptoms of dyspnea. This method is worthy of clinical promotion.
