摘要
目的探讨妊娠期高血压疾病(HDCP)对新生儿肾功能及脐血胆红素水平的影响,为改善新生儿生命质量提供依据。方法选取2017年7月~2018年6月瑞安市人民医院妇幼分院收治的HDCP患者及同期在该院生产的健康孕妇各200例,均以孕妇分娩的新生儿为研究对象,分别设为观察组和对照组。于新生儿出生后立即抽脐血5ml,检测血清肌酐(Cr)、尿素氮(BUN)、尿酸(UA)、β2微球蛋白(β2-MG)及胆红素水平(T-BIL)。结果 1、观察组新生儿血Cr、BUN、UA、β2-MG及T-BIL水平均高于对照组新生儿,差异有统计学意义(t=7.888、6.765、8.334、6.345、3.548,P<0.05)。2、新生儿血Cr、BUN、UA、β2-MG及T-BIL水平与母亲妊娠期血压呈正相关(r=0.421、0.341、0.391、0.326、0.177,P<0.05)。结论 HDCP孕妇分娩的新生儿肾功能较正常孕妇分娩的新生儿低下,脐血胆红素水平较正常孕妇分娩的新生儿高,且呈线性相关,即母亲血压越高新生儿肾功能指标及脐血胆红素水平越高。HDCP与新生儿肾功能损伤及新生儿高胆红素血症发生有关,临床上应及早给予干预。
Objective: To explore the influence of hypertensive disorders complicating pregnancy (HDCP) on the renal function and Cord blood Bilirubin of newborns and to provide the basis for improving the quality of newborn' s life. Methods: We recruited 200 HDCP patients who received treatment in Ruian people's hospital maternity and child branch from July 2017 to June 2018 as the observation group. Another 200 healthy pregnant women were enrolled in the same period as the control group. The newborns delivered by the recruited pregnant women were enrolled as the researching subjects. We sampled 5ml Umbilical cord blood from each newborn immediately after birth, and ~E-MG, Cr, BUN, UA and T-BIL of the newborns were detected. Results: 1.The levels of Cr, BUN, UA, {32-MG and T-BIL in the observation group were higher than those in the control group, the differences were statistically significant (t=7.888, 6.765, 8.334, 6.345, 3.548, P〈0.05) . 2. The levels of serum Cr, BUN, UA, ^-MG and T-BIL in neonates was positively correlated with the severity of maternal hypertension (r=0.421. 0.341, 0.391, 0.326, 0.177, P〈0.05) . Conclusion: The newborns delivered by HDCP patients have poorer renal function than the newborns delivered by healthy pregnant women. And the level of T-BIL in cord blood of the newborns delivered by HDCP patients is higher than that in newborns delivered by normal pregnant women. The higher the maternal blood pressure, the higher the renal function and the level of T-BIL in neonates. HDCP is associated with neonatal renal dysfunction and neonatal hyperbilirubinemia. So early intervention should be given to the clinic.
