胸腺法新对社区获得性肺炎患者凝血指标、免疫功能、PCT及sTREM-1水平的影响

Effect of thymalfasin on coagulation index,immune function,PCT and sTREM-1 levels in patients with community acquired pneumonia

目的:探讨胸腺法新对社区获得性肺炎患者凝血指标、免疫功能、降钙素原(procalcitonin,PCT)及可溶性髓样细胞激发受体(triggering receptor expressed by myeloid cells-1,sTREM-1)水平的影响。方法:选取于本院治疗的160例社区获得性肺炎患者纳入研究,随机分为对照组80例和观察组80例。对照组患者给予止咳平喘、抗菌等常规治疗,观察组患者在常规治疗基础上给予胸腺法新治疗。分别于治疗前后检测两组患者血清中D-二聚体、纤维蛋白原(fibrinogen,FBG)、补体C3、补体C4、T淋巴细胞亚群、PCT及sTREM-1水平。结果:治疗前,两组血清中D-二聚体、FBG、C3、C4、CD3^+、CD4^+、CD8^+、CD4^+/CD8^+、PCT及sTREM-1水平比较,均无统计学差异(P>0.05)。治疗后:对照组及观察组CD3^+、CD4^+及CD4^+/CD8^+水平较治疗前均显著升高,D-二聚体、FBG、CD8^+、PCT及sTREM-1水平较治疗前均显著降低;且治疗后观察组CD3^+、CD4^+及CD4^+/CD8^+水平明显高于对照组,D-二聚体、FBG、CD8^+、PCT及sTREM-1水平明显低于对照组,差异有统计学意义(P<0.05)。结论:胸腺法新治疗社区获得性肺炎患者有一定的临床效果,可显著改善患者凝血功能,增强免疫能力,并缓解炎症状态。

Objective： To explore the effect of thymalfasin on coagulation index, immune function, PCT and sTREM-1 levels in patients with community acquired pneumonia. Methods： A total of 160 patients with community acquired pneumonia admitted in our hospital from June 2016 to June 2017 were divided into the control group and the observation group according to the random number table method, 80 cases in each group. The control group was given conventional treatment, while the observation group was given thymalfasin on the basis of conventional treatment. The levels of D-D, FBG, complement C3, C4, T lymphocyte subsets, PCT and sTREM-1 were detected before and after treatment. Results： Before treatment, there was no statistical difference between the serum levels of D-D, FBG, C3, C4, CD3 ＋, CD4 ＋, CD8 ＋, CD4 ＋/CD8 ＋, PCT and sTREM-1 in the two groups of patients （ P 〉0.05）. After treatment, the levels of CD3 ＋, CD4 ＋ and CD4 ＋/CD8 ＋ were significantly increased, and the levels of D-D, FBG, CD8 ＋, PCT and sTREM-1 were significantly decreased in the two groups, and the level of CD3 ＋, CD4 ＋ and CD4 ＋/CD8 ＋ in the observation group was significantly higher than that of the control group after treatment, and the D-D, FBG, CD8 ＋, PCT and sTREM-1 levels were significantly lower than those in the control group （ P 〈0.05）. Conclusion： Thymalfasin has a certain clinical effect in the treatment of patients with community acquired pneumonia. It can significantly improve the blood coagulation function, enhance the immune ability and alleviate the inflammatory state.
