2型糖尿病患者血清瘦素水平及其临床意义

THE OBSERVATION OF SERUM LEPTIN IN TYPE Ⅱ DIABETES MELLITUS AND ITS CLINICAL SIGNIFICANCE

目的 观察 2型糖尿病 (DM )患者血清中瘦素的变化水平 ,探讨瘦素、胰岛素与 2型DM之间的相互关系。方法 采用放射免疫法对 2 0 0例 2型DM患者和 14 0例正常对照者血清中瘦素和胰岛素进行测定。结果 空腹血清瘦素 2型DM无肥胖者为 (6 .96± 3.17) μg/L ,肥胖者为 (15 .4 2± 8.4 6 ) μg/L ;正常对照组无肥胖者为 (5 .0 7± 2 .0 5 ) μg/L ,肥胖者为 (14 .11± 6 .2 6 ) μg/L。 2型DM患者血清瘦素水平的变化与健康者基本一致。无论是非肥胖者 ,还是单纯肥胖者 ,血清瘦素水平均存在明显性别差异 ,女性血清瘦素水平高于男性 2～ 3倍。但其与年龄分布之间的关系不明显。血清瘦素水平与空腹胰岛素水平呈正相关。结论 2型DM患者餐后 2h血清瘦素水平与基础值无差异 ,说明 2型DM患者不存在ob基因缺陷 ,亦无瘦素受体缺陷 ,瘦素可能不是导致

Objective To observe the changes in serum leptin in patients with Type Ⅱ diabetes mellitus and to explore the correlation between serum leptin,insulin and Type Ⅱ diabetes mellitus.Methods Radio immunity techniques were used to detect the serum leptin in 200 cases of Type Ⅱ diabetes mellitus and 140 normal cases.Results The serum leptin was(15.42±8.46)μg/L in fat patients with Type Ⅱ diabetes mellitus and (6.96±3.17)μg/L in other patients.The serum leptin was(14.11±6.26)μg/L in fat cases in the contrast group and(5.07±2.05)μg/L in other cases of this group.No significant differences were found between the two groups.Obvious differences in serum leptin existed in simple obesity cases and other cases of normal build.The serum leptin level in females was two or three times higher than that in males.There seemed to be no obvious correlation between serum leptin and age.Positive correlation was found between serum leptin and fasting insulin levels.Conclusion There existed no difference in serum leptin levels at normal time and two hours later after meal,which indicates that no ob defects or leptin receptor defects exist in pattents with Type Ⅱ diabetes mellitus.Serum leptin may not be the major factor resulting in Type Ⅱ diebetes mellitus.
