Hunt-Hess高分级破裂脑动脉瘤的急性期手术治疗

Emergent Surgery of Hunt-Hess High Graded Ruptured Intracranial Aneurysm

目的 探讨Hunt-Hess高分级破裂脑动脉瘤的急性期手术治疗效果。方法 对1995年1月～2000年12月间29例Hunt—Hess高分级破裂动脉瘤患者的急性期手术治疗情况手术效果进行回顾性分析。其中Ⅳ级27例,Ⅴ级2例。结果 随访2个月～4年,本组患者生存率为83％;恢复良好者7例,皆为Hunt Ⅳ级病例,占24％;Hunt Ⅴ级病例无一例恢复良好,预后不佳。结论 对Hunt Ⅳ级破裂动脉瘤应提倡急性期手术,而对于Ⅴ级患者应避免急性期手术。

To assess the efficacy of emergent surgery for Hunt- Hess high graded rup-tured intracranial aneurysm. Methods Twenty nine cases from January,1995 to December,2000 were retrospectively analyzed according to their surgery effect. There were 27 cases in grade IV , 2 cases in grade V . Results Follow up was made from 2 raonths to 4 years, 7 cases had good recovery, all in grade N ; no grade V case had good recovery and all had poor prognosis. Conclnsions Emergent surgery should be done on grade IV cases but not on grade V cases.
