肩关节脱位:MR以及MR关节造影评价 被引量:5

Anterior Shoulder Dislocation: Evaluation with MR and MR Arthrography

摘要 目的 总结肩关节前方脱位的MR表现。方法 回顾性分析 2 6例肩关节前方脱位或复发性脱位患者的常规MR和MR关节造影图像 ,重点观察肩关节骨结构、前下关节盂唇、和前下关节囊情况。结果  2 6例患者中 ,Hill sachs病变的出现率为 77% (2 0 /2 6) ,前下关节盂唇异常的出现率为 88% (2 3 /2 6) ,前下关节囊附着II型和III型占 19% (5 /2 6) ,前下关节盂窝骨折占 8% (2 /2 6)。MR关节造影对盂唇和关节囊的评价优于常规MR。 3例外展外旋位MR造影显示前下盂唇撕裂优于MR关节造影。结论 肩关节前方脱位主要表现为前下关节盂唇异常和Hill sachs病变 ,MR关节造影更有助于诊断前下盂唇病变。 Objective To summary the MR findings of anterior shoulder dislocation. Methods The routine MR images and MR arthrography images of 26 patients with anterior shoulder dislocation or recurrent dislocation were retrospectively analyzed. The emphasis was focused on the bone structure of the shoulder, the anteroinferior labrum, and the anterior capsular insertion type.Results The Hill sachs lesion was found in 77%(20/26)patients and abnormality of the anteroinferior labrum (Bankart lesion) was seen in 88%(23/26). Type II and type III capsular insertions were only found in 19%(5/26)of all patients, and the presence of osseous bankart lesion was only 8%(2/26). In revealing the abnormalities of the labrum and capsular, MR arthrography was superior to routing MR. Conclusion The main MR appearances of anterior shoulder dislocation were abnormalities of the anterior labrums and Hill sachs lesions. MR arthrography is more helpful to diagnose the abnormalities of anteroinferior labrum than routing MR examination.
