脑损害患者不同形式对消与忽视的相互关系及脑机制

The Brain Mechanism and Relationship between Different Kinds of Extinction and Neglect in Brain Damaged Patients

目的 :探讨不同形式对消之间及忽视与对消之间的相互关系 ,对左及右半球损害之后对消、忽视的发生率及程度 ,及探讨不同形式对消与忽视的脑机制 .方法 :针对健康对照组、右脑损害组、左脑损害组进行数字、词汇言语性分听测验 ,简单声响分听测验、触觉性双侧同时刺激两项非言语性对消测验 .同时对3组被试进行了线段划消、复杂多形划消两项视觉空间忽视测验 .结果 :(1)在RBD组与LBD组间 6项测验检出对消、忽视无一致性结果 ,远较对消是一种轻度忽视或是一种传导性障碍的意见要复杂 ;(2 )两组间数字、词汇分听测验的对消程度未见显著差异 ,反常对消仅见于LBD组 .RBD组忽视、对消的严重程度均强于LBD组 ;(3)RBD组的反常性对消及同侧忽视也高于LBD组 .结论 :对消和忽视、不同形式的对消或不同形式的忽视可能存在不同的脑机制 ,并非单一假说能够解释 .

Objective: To explored brain mechanism of extinction and neglect. Methods: There are different tests for examining extinction and neglect, of which is the word dichotic listening tests (need the verbal medium). However, the simple sounds dichotic listening, double simultaneous stimulation of tactile and 2 visual spatial neglect tests do not need the verbal medium. All tests were performed on healthy, RBD and LBD subjects. Results: We analyzed the relationship between every 2 tests among all tests and 3 subject groups, and explored brain mechanism of extinction and neglect. Conclusion: Extinction and neglect, and different types of extictiong and neglect may exist different brain mechanism, no simple relationship between them.
