经腹超声检查和64排螺旋CT在胃癌术前T分期中的诊断价值比较

Comparison of the Diagnostic Value between Transabdominal Ultrasonography and 64-Slice Spiral CT in Preoperative T Staging of Gastric Cancer

目的探讨经腹超声检查和64排螺旋CT(MSCT)在胃癌术前T分期中的诊断价值。方法回顾性分析2015年2月-2017年2月本院收治的124例胃癌患者临床资料,依据其术前行经腹超声检查(记为超声组,n=60)和64排螺旋CT检查(记为MSCT组,n=64)分为两组,将两组诊断胃癌术前分期结果与术后病理结果进行对照,比较经腹超声检查和MSCT在胃癌术前T分期中的诊断价值。结果胃癌患者经腹超声检查和多层螺旋CT检查的影像学表现各具典型特征,以术后病理结果为金标准,64排螺旋CT对胃癌术前T分期中T1、T2、T3、T4准确率86.67%、77.78%、84.00%、81.82%较经腹超声对胃癌术前T分期中T1、T2、T3、T4准确率60.00%、55.56%、72.00%、63.64%明显高,且64排螺旋CT对胃癌术前T分期诊断与术后病理结果的一致性较好。结论 64排螺旋CT在胃癌术前T分期诊断中的准确率较经腹超声检查明显高,更适于临床推广应用。

Objective To investigate the diagnostic value of transabdominal ultrasonography and 64-slice spiral CT(MSCT)in preoperative T staging of gastric cancer.Methods The clinical data of 124 patients with gastric cancer admitted to the hospital from February2015 to February 2017 were retrospectively analyzed.Patients undergoing preoperative ultrasonography were included in the ultrasound group(n=60)while those undergoing64-slice spiral CT were included in the MSCT group(n=64).The preoperative staging results of the two groups were compared with postoperative pathological results.The diagnostic value of transabdominal ultrasonography and MSCT in the preoperative T staging of gastric cancer was compared.Results With postoperative pathological findings as the golden standard,the accuracy rates of 64-slice spiral CT in preoperative T staging of T1,T2,T3 and T4(86.67%,77.78%,84.00%,81.82%)were significantly higher than those of transabdominal ultrasonography(60.00%,55.56%,72.00%,63.64%).The consistency between CT and postoperative pathological results in preoperative T staging was better.Conclusion The accuracy of 64-slice spiral CT in the preoperative T staging of gastric cancer is significantly higher than that of transabdominal ultrasonography.It is more suitable for clinical application.
