锥形束CT在口腔正畸埋伏阻生牙诊疗中的应用 被引量:12

摘要 目的探究锥形束CT(CBCT)在口腔正畸埋伏阻生牙诊疗中的应用价值。方法选择2017年1月-2018年10月于我院就诊并行正畸治疗的45例患者进行回顾性分析,均行X线根尖片及CBCT定位检查,比较两种检查方法对上颌埋伏阻生牙的定位及矫治方案的指导价值。结果 45例患者均为上颌埋伏牙,共计63颗牙。CBCT术中符合率100.0%显著高于X线根尖片79.4%,差异有统计学意义(P<0.05)。CBCT可对颌骨内埋伏阻生牙准确定位,且与其实际情况完全一致,手术入路均准确无误,创口大小适合。45例患者中36例行正畸牵引治疗,9例因位置倒置或畸形拔除埋伏阻生牙及多生牙。结论 CBCT可在三维方向上直观反映阻生牙的位置、形态、方向及与邻牙的关系,对上颌埋伏阻生牙的定位及矫治方案的准确制定具重要指导价值。 Objective To explore the value of cone beam computed tomography(CBCT)in the diagnosis and orthodontic treatment of embedded and impacted teeth.Methods A retrospective analysis was performed on 45 patients who underwent orthodontic treatment in the hospital from January 2017 to October 2018.All patients underwent X-ray periapical film and CBCT localization and the clinical value was compared between the two.Results A total of 45 patients had maxillary impacted teeth.The coincidence rate of CBCT was higher than that of X-ray periapical film(100.0%vs 79.4%)(P<0.05).CBCT can accurately locate the impacted teeth in the jaw,and the result was completely consistent with the actual situation.The surgical approach was accurate and the wound size was suitable.Of the 46 patients,36 underwent orthodontic traction and 9 underwent removal of the embedded and impacted teeth due to inversion or malformation.Conclusion CBCT can visually reflect the position,shape,direction and relationship with adjacent teeth in three-dimensional direction.It is of great guiding value for the positioning of the maxillary impacted teeth and the accurate formulation of the correction plan.
