外伤性感染性眼内炎病因和致病菌临床分析

Clinical analysis of etiology and pathogen of infectious endophthalmitis.

目的 了解感染性眼内炎流行病学特点及动态变化。方法 回顾性分析 5年内 2 13例感染性眼内炎住院患者的资料 ,对病因及其致病菌进行统计分析。结果 眼外伤是眼内炎的主要致病原因占 85 .92 % ;在儿童组占 98.67% ,其中一次性废弃注射器致伤者最多占 3 3 .3 3 %。内眼手术后感染主要发生在老年组占 2 9.2 7%。葡萄球菌和革兰阴性杆菌是主要致病细菌 ,分别占 48.49%和 2 5 .3 5 % ;真菌占 2 1.2 3 %。细菌感染对环丙沙星敏感者最多 ,为 96.0 8% ,头孢唑啉次之。经治疗后92 .49%的患者炎症得到控制 ,视力不同程度提高。结论 应重视眼外伤预防 ,加强一次性注射器管理。早期诊断。

Objective To study the epidemiological and clinical characters of infectious endophthalmitis.Methotls Medical history records of 213 cases of infectious endophthalmitis within 5 years were analyzed retrospectively.Results Ocular trauma was the main cause of endophthalmitis accounting for 85.92%,in children group,trauma accounting for 98.67% and most of them caused by disused tip of syringes(33.33%).Postoperative endophthalmitis occured in aged group.Staphylococcus, bacilli and fungus were the main pathogens (48.49%, 25.35%and 21.23%). The most sensitive antibiotics was ciproflaxacin.Conclusions The key point of the treatment was prevention.Intravitreus injection of antibiotics and vitrectomy in time may be the most valid way of diagnosis and treatment.
