严重开放性眼损伤的眼前段重建术治疗

Treatment of severe opened eye injury with anterior segment ocular reconstruction.

目的 探讨严重开放性眼前段损伤进行眼前段重建术治疗的方法 ,并观察其治疗效果。方法 严重眼前段开放性损伤 1 2例 1 2眼 (眼球破裂伤 5例、撕裂伤 4例、穿孔伤 3例 )采用虹膜切除、晶状体摘出、玻璃体切除、角膜移植和重建眼球形态等联合手术进行治疗。结果 术后随访 1～ 2年 ,1 2例中 1 1例保全眼球 ,7例角膜移植片透明 ,5例获得 0 0 5以上有用视力。主要并发症有角膜移植排斥反应 (5例 )及虹膜出血 (4例 )。结论 眼前段重建术是治疗严重开放性眼前段损伤唯一行之有效的方法。

Objective To investigate the technigue of anterior segment reconstruction and its results in severe opened eye injury.Methods 12 cases(12 eyes) of severe anterior opened eye injury,including 5 cases of ocular rupture,4 cases of ocular laceration,3 cases of penetrating ocular injury,were treated with combined surgery of irisectomy,lensectomy,vitrectomy, corneal transplantation.Results After 1～2 years' follow-up,11 of the 12 eyes were saved,7 cases with clear crneal grafts, and 5 cases got 0.05 or better of vision.The main complications were corneal grafts rejection (5 cases) and iris hemorrhage (4 cases).Conclusion Anterior segment ocular reconstruction is the only effective method in the treatment of severe opened anterior ocular injury.
