老年性髋部骨折的手术治疗

The Surgery Treatment of gerontal Coxa Fracture

目的 评价 12 7例老年性髋部骨折手术治疗的效果。方法 根据老年人的全身情况和骨折的类型 ,分别选择手术方式是 :粗隆间骨折固定用多根螺纹钉 19例、加压滑动鹅头钉 13例、Camma钉 4例。股骨颈骨折用螺旋钉 3 5例 ,多根螺纹钉 2 0例 ,加压滑动鹅颈钉 5例 ,人工股骨头置换术 16例、人工全髋关节置换术 15例。结果 经过平均 2 3个月的随访 ,优良率 :粗隆间骨折 63 .3 % ;股骨颈骨折内固定组 5 3 .9% ,假体置换组 85 .7%。结论 对老年性髋部骨折 ,采取积极和谨慎的合适的手术治疗方式 ,能使病人尽早离床活动 ,恢复功能和自理能力 ,减少卧床带来的并发症。取得了较好的疗效。

Objective: To evaluation the effect of surgery treatment of gerontism coxa fracture. Methods: Thetype of operation was detemined by status of body and type of coxa fracture. Multiscrew posts were used to fixed intertrochanteric fraction in 19 patients,pressor sliding hip nails were used in 13 patients,and Gamma nails were used in 4 patients.Spiral nail was used to fixed fracture of neck of femur in 35 patients,multi screw posts were used in 20 patients,pressor slideng goose neck nails were used in 5 patinets,artificial replacement for head arthroplasty was used in 16 patients,and artificial replacement for hip joint was used in 15 patients. Results: The rate of effect was 63.5% in group of intertrochanteric fraction,53.9% in group of fracture of neck of femur with interal fixation,and 85.7% in group of prosthesis replacement. Conclusion: The suitable operation mod should be excerpted for gerontal coxa fracture. It can acceleration patients activity out of bed,recove function of bone and arthroand selfcare ability,and reduce the complication of laying in bed. It take better treatment effect.
