高血压左室肥大对冠状动脉基础血流和储备功能影响的探讨 被引量:2

摘要 目的 探讨高血压及左室肥大对冠状动脉基础血流和储备功能的影响。方法 通过多平面食道多普勒超声分别测定左冠状动脉主干静息状态下和静脉注射潘生丁后舒张期峰值血流速度(PDV)和平均血液速度(MDV),计算静脉注射潘生丁后与静息状态下上述测值之比值D/R,比较高血压左室肥大组(C组)与单纯高血压组(B组)及正常组(A组)各测算值。结果 D/R值C组和B组均显著低于A组(P<0.05~0.01),其中,静脉注射潘生丁后C组PDV和MDV值显著低于B组和A组(P<0.05~0.01),后二组之间接近,而静息状态下B组PDV和MDV值显著高于A组和C组(P<0.05~0.01)。结论 高血压左室肥大患者冠状动脉基础流速及储备功能均下降。 Objective To investigate the effect of hypertension and hypertensive left ventricular hypertrophy on coronary basic flow velocity and coronary flow reserve. Methods Coronary peak diastolic flow velocity (PDV) and mean diastolic flow velocity (MDV) of left main coronary were measured by multipe planes transesophageal doppler echocardio-graphy in rest status and after intravenous injection of dipyradimole respectively. The ration of above parameters to that in rest status (D/R) was calculated after intravenous injection of dipyradimole. Comparison among normal group (group A), pure hypertension group (group B) and hypertension with left ventricular hypertrophy group (group C) was made. Results D/R ratio of group B and C was significantly lower than that in group A (P<0. 05-0. 01). After intravenous injection of dipyradimole, PDV and MDV of group C were significantly lower than those of group B and A (P<0. 05-0. 01 ). While in rest status PDV and MDV of group B were significantly higher than those of group A and C (P<0. 05-0. 01). Conclusion Both the coronary basic flow velocity and coronary flow reserve in hypertensive patients with left ventricular hypertrophy were reduced.
