摘要
目的 :研究胚胎发育不同时期胆总管壁上皮细胞凋亡与增殖情况。方法 :取不同胎龄人工流产胎儿胆总管标本各 5例均经光镜下观察病理变化 ,用免疫组织化学法检测细胞凋亡相关指标bcl 2、bax及细胞增殖指标PCNA表达情况 ,用TUNEL法检测细胞凋亡情况。结果 :胎儿胆管上皮组织完整 ,多为柱状上皮细胞 ,可见少量平滑肌细胞分布于纤维组织层中 ,随胎龄增长纤维层增厚 ,无明显炎症反应。胚胎发育的 6个月、7个月、8个月、9个月及 10个月胆总管上皮均存在细胞凋亡与增殖 ,6个月胎儿胆总管上皮细胞凋亡率、增殖指数及BAX的阳性率最高 ,而bcl 2的阳性率最低 ,随胎龄的增加胆总管上皮细胞凋亡率、增殖指数及BAX的阳性率逐渐降低 ,bcl 2的阳性率随胎龄的增加逐渐升高 ,胆总管上皮细胞凋亡率、增殖指数在各月份之间差异有显著性 (P <0 .0 5 )。结论 :细胞凋亡与增殖的动态平衡在正常胆管发育过程中起着重要作用。
Objective:To observe apoptosis and proliferation of choledochus wall epithelial cell in the different period of embryo growth and to understand the effects of apoptosis and proliferation on choledochal cyst development.Methods:Choledochus specimens were gained from 25 artificial abortion fetus,whose age was 6?7?8?9?10 months separately. All specimens were observed under the light microscope. The apoptosis related index(bcl 2 and bax) and cell proliferation index(PCNA) were detected by the immunohistochemical technique;apoptosis was detected by TUNEL method.Results:Bile duct epithelial tissue cell is integrated,with columnar epithelial cell. A few smooth muscle cells distributing in the fiber layer were visible,and the fiber layer became thicker with the increase of age,and no clear inflammative reaction was found. Apoptosis and proliferation were present in bile duct epithelial cell in fetal development(over 6?7?8?9?10 months old). Apoptosis ratio,proliferating index and BAX positive ratio were the lowest while bcl 2 positive ratio was the highest in the 6 month old tetus(P<0.05).Conclusion:Dynamic balance between cell proliferation and apoptosis were involved in the normal development of bile ducts.
