内踝上皮支皮瓣的临床应用

THE CLINICAL APPLICATION OF SKIN FLAP PEDICLED WITH UPPER VESSEL OF MEDIAL MALLEOLUS

目的 :探讨内踝上皮支皮瓣的临床应用价值。方法 :设计应用内踝上皮支皮瓣 6例。皮肤及软组织缺损部位 :同侧踝部 2例 ,同侧足跟部 2例 ,对侧足背部 1例 ,对侧内踝部 1例。皮肤及软组织缺损面积 :3cm× 4cm～ 7cm× 1 0 cm。切取内踝上皮支皮瓣面积 :4cm× 7cm～ 8cm× 1 7cm。结果 :皮瓣全部成活 5例 ,皮瓣远端皮肤部分坏死 1例 ,经换药痂下愈合。结论 :内踝上皮支皮瓣局部转移操作简单 ,成活率高 ,是修复同侧或对侧下段。

Objective:To investigate the clinical application of the flap pedicled with upper vessel of medial malleolus.Methods:Six skin flaps were designed to treat six cases of skin and soft tissue defects,which included 2 on the malleolus and 2 on the heels of the same side,1 on the instep and 1 on the medial malleolus of the opposite.The areas of the defects varied from 3 cm×4 cm to 7 cm×10 cm,and those of the flaps varied from 4 cm×7 cm to 8 cm×17 cm.Results:5 flaps survived completely.In one case,the distal skin of the flap necrotised partly and heeled under scar after continuous dressing.Conclusion:Easy to operate and with a high survival rate,the flap pedicled with upper vessel of medial malleolus was an excellent method to treat the skin and soft defect on the distal legs,malleolus and heels of either side.
