小脑凸面脑膜瘤的手术治疗

Surgical treatment for cerebellar convexity meningiomas.

目的探讨小脑凸面脑膜瘤的诊断和手术治疗。方法回顾分析 9例小脑凸面脑膜瘤患者的临床表现、影像学特征、手术方法和治疗结果。结果按肿瘤部位将其分为三种类型 :中间型、外侧型和上部型 ,分别为 2例、4例和 3例。均行手术切除 ,其中 Simpson I级切除 3例 , 级切除 4例 , 级切除 2例。术后症状改善者 8例 ,1例因脑积水行脑室 -腹腔分流术。 2例术后复发再次行手术切除。结论术前影像学检查有助于小脑凸面脑膜瘤的诊断和手术治疗 ,根据肿瘤的类型采取相应的手术方式 ,术中应注意静脉窦。

Objective To investigate the diagnosis and surgical treatment of cerebellar convexity meningiomas.Methods Clinical data of 9 cases with cerebellar convexity meningiomas,including clinical manifestation and preoperative neuroimaging,surgical treatment and results were retrospectively analyzed.Results The tumor was divided into three types according to the location:medial type in 2 cases,lateral type in 4 cases and upper type in 3 cases.The tumors were all removed including Simpson I removal in 3 cases,II in 4 cases and III in 2 cases.After operation,8 cases were improved,1 was operated on ventricule peritoneal cavity shunting because of hydrocephalus,and 2 cases underwent removal again because of relapse.Conclusion Preoperative imaging examination is helpful to the diagnosis and surgical treatment of cerebellar convexity meningiomas.Based on the tumor types,surgical approaches are selected.The management of venous sinus and hemorrhage needs close attention during the operation.
