肾及输尿管上段手术体位固定方法的改进

Improvement on Fixation Method of Operative Posture in Patients Underwent Renal and Upper Segment of Ureter Operation

目的 :为满足肾及输尿管上段手术体位要求。方法 :对施行肾及输尿管上段手术的病人 430例 ,随机分为两组 ,A组采用新的固定方法 ,一侧用骨盆固定器加海绵垫和束脚带固定方法 ,B组采用传统的固定方法即两侧都用骨盆固定器加海绵垫和束脚带固定方法。观察两组固定体位时间 ;固定体位后病人舒适情况、固定后体位的稳定性及对操作的影响等情况。结果 :两组固定体位时间经统计学分析 ,t =2 2 .48,P <0 .0 1;病人舒适率t =11.87,P <0 .0 1;两种方法固定后体位的稳定性及对操作的影响 ,经四格表精确概率统计 ,P <0 .0 1。结论 :新的固定法优于传统固定法 ,固定体位后体位端正 ,不影响消毒范围和操作 ,而且省时。

Objective: to meet the needs for operative posture of patients underwent renal and upper segment of ureter operation. Methods: a toal of 430 cases who were to undergo renal and upper segment of ureter operation were divided randomly into A and B groups. Patients of group A adopted the new way of fixation which applied pelvic fixator plus foam-rubber cushion and foot girdle unilaterally. While patients of group B were just given traditional way of fixation. Items such as time consuming of fixation, comfortable feeling of patients, the stability of posture after fixation, and the affect on the operation of patients were observed. Results: in terms of fixation time and comfortableness rate of patients, there were statistical significance between the two groups (t=22.48, P<0.01; t=11.87, P<0.01). In terms of the stability of posture after two different ways of fixation and their affects on operation, there were also statistical differences (P<0.01).Conclusion:the new fixation method was superior to the traditional way of fixation. It has the advantage of saving time and labor, and has no impact on disinfection and operation itself.
