大剂量盐酸氨溴索对ICU中ARDS患者脱机时间的影响 被引量:11

摘要 目的:研究大剂量盐酸氨溴索对急性呼吸窘迫综合征(acute respiratory distress syndrome,ARDS)患者脱离呼吸机时间的影响。方法:选择ARDS患者38例,随机分成观察组(A组,n=19)和对照组(B组,n=19)。A组每天给予盐酸氨溴索20 mg/kg加生理盐水500 mL静脉滴注,B组每天给予单纯生理盐水500 mL静脉滴注,连续应用7 d。比较2组患者在呼吸机参数、血气指标、肺损伤程度和血清超氧化物歧酶(SOD)活力等方面的变化以及患者撤离呼吸机时间上的差异。结果:用药后A组患者的氧合及呼吸力学指标、肺损伤评分、SOD值均明显改善,脱机时间明显较B组短,与B组比较均有统计学差异(P<0.01或0.05)。结论:临床应用大剂量盐酸氨溴索有利于ARDS患者早日脱离呼吸机治疗。 Objective:To evaluate the effect of high dose ambroxo1 on the time of weaning from mechanical ventilation of patients with ARDS in ICU.Methods:38 patients with ARDS were divided randomly into 2 groups,Patients in group A were treated with high dose ambroxol(20 mg/kg/d)+0.9%NaCL(500 mL),and those in group B received 0.9%NaCL(500 mL) alone for 7 consecutive days.The SOD measurement and the parameters of respiratory mechanics and the arterial blood samples were taken for blood gas were recorded before ambroxol infusion,7 days following the first infusion respectively.Outcome of patients in both groups including various pulmonary function indices was analyzed.Results:The PaO2,SPO2,PaO2/FiO2 and CL of pa-tients in group A with high dose ambroxol therapy were improved significantly(P<0.01),which were much better than those in group B.But RR,f/Vt,PIP and RAW in group B were higher than those in group A(P<0.01),and the SOD activity of the patients in group A were improved significantly(P<0.01).The time of weaning from mechanical ventilation of patients in group A was less than that group B(P<0.01).Conclusion:The results suggest that high dose ambroxol could shorten the time of weaning from mechanical ventilation of patients with ARDS in ICU
