球周注射曲安奈德治疗甲状腺相关性眼病的临床观察

Clinical observation of Triamcinolone peribulbar injection on thyroid associated ophthalmopathy

目的:探讨球周注射曲安奈德(TA)治疗甲状腺相关性眼病(TAO)的疗效。方法:对30例(46眼)甲状腺相关性眼病患者行曲安奈德20 mg加2%利多卡因0.5 mL球周注射,随访观察并记录患者自觉症状、眼部体征情况。结果:所有患者经治疗后畏光、流泪、异物感等刺激症状消失或明显减轻,46眼眼球突出患者中回退(≥2 mm)的有39眼,有效率为84.78%。32眼存在复视及眼球运动受限的患者有11眼消失,占34.37%;有15眼改善,占46.87%。36眼睑裂高度睑裂高度增大的患者有32眼回缩,回缩率为88.88%。结论:曲安奈德球周注射治疗TAO是一种安全有效的治疗方法,无严重糖皮质激素全身副作用和局部注射并发症发生。

Objective:To investigate the effect of the peribulbar injection of triamcinolone acetonide on thyroid associated ophthalmopathy(TAO).Methods:30 cases(46 eyes) with thyroid-associated ophthalmopathy were treated with an peribulbar injection of triamcinolone acetonide 20 mg plus 2% lidocaine 0.5 ml,Observe and record the follow-up of patients with symptoms,signs of eye.Results:All patients after treatment photophobia,tearing,foreign body sensation and other symptoms disappeared or significantly reduce the stimulation,46 exophthalmos in patients with regression(≥ 2 mm) were 39,efficiency of 84.78%.32 there is diplopia and limited eye movement disappeared 11 patients,accounting for 34.37%;a 15 improvement,accounting for 46.87%.36 eyelid fissure height of palpebral fissure height increase of the patients had 32 retraction,retraction rate of 88.88%.Conclusion:peribulbar injection of triamcinolone acetonide was a safe and effective treatment of TAO,no severe systemic side-effects of glucocorticoid and local injection of complications.
