CDE对干下型室间隔缺损、主动脉窦瘤的误诊原因分析

Analysis of Erroneous Diagnosis on the Ventricular Septal Defect of Stem - Inferior Type by CDE.

目的:分析彩色多普勒超声心动图(CDE)对先天性心脏病诊断的漏诊、误诊情况,提高确诊率。材料与方法:21例先天性心脏病患者,男11例,女10例,进行CDE与手术结果对照。结果:CDE对干下型室间隔缺损漏诊率为11％;对右窦瘤的误诊率为50％;对窦瘤破裂的误诊率为81％;对主动脉瓣关闭不全或脱垂的诊断符合率为100％。结论:超声探测时应仔细调整声束角度,多方了解合并畸形,明确分流信号,方可正确诊断。

Purpose: To analyze the causes of leaky diagnosis and/or erroneous diagnosis on the congenital cardiopathy by Color Doppler Echocardiography (CDE) and elevate the diagnostic accuracy. Materials and Methods: Results of CDE in 21 patients with congenital cardiopathy including 11male and 10 female have been compared with that of the operations. Results: For the rate of erroneous diagnosis generated from CDE, it was defferent with types of the diseases, for example, that of stem - inferior type’ s ventricular septal defect was 11%, right aortic sinus tumor was 50%, and the rupture of aortic sinus tumor was 81%. When CDE was used for the diagnoses of patients with aortic insufficiency or aortic valval pto-sis, the coincidence rate of the diagnosis was 100%. Conclusion: In order to obtain the accurate diagnosis of patients with various congenital cardiopathy, it will very important to regularize carefully the angle of sound beam, to understand completely the combined malformation and to clear the signal of shunt.
