超声显像与CT对阻塞性 黄疸诊断的准确性比较(附56例病例分析)

Comparison of the Diagnostic Accuracy between Ultrasonography and Computed Tomography for the Obstructive Jaundice.

目的:比较超声显像与CT诊断阻塞性黄疸的准确性,寻求最佳方法。材料与方法:本文讨论了56例经手术与病理证实为阻塞性黄疸的超声检查特点,并与CT诊断进行了比较。结果:良性病变28例,其中胆管结石18例,占64.1％,超声对其诊断符合率,与CT相同,均为100％。恶性病变28例,其中胆管癌17例,胰头癌6例,共占恶性病变的82.1％,超声对其诊断的符合率为100％,而CT的诊断符合率为95.7％。就梗阻部位的诊断符合率而言,超声高于CT,分别为92.9％(52/56)和85.7％(48/56)。对梗阻病因的诊断符合率也是超声高于CT,即分别为89.3％(50/56)和80.4％(45/56)。结论:超声显像对阻塞性黄疸的诊断明显优于CT,可作为诊断阻塞性黄疸的首选方法。

Purpose: To select an optimal program from the comparison of the diagnostic accuracy between ultrasonography and computed tomography for the obstructive jaundice. Materials and Methods: The characteristics and the diagnostic accuracy of ultrasonography in 56 cases of patients with obstructive jaundice confirmed by the operation and the pathology have been discussed and compared with computed tomography. Results: The accuracy or the rate conformable to pathologic diagnosis of ultrasonography in the 28 cases of patients with benign disease which included 18 cases of extrahepatic cholelithiasis was like as that of computed tomography, i. e. the both have achieved to 100% , but the accuracy between ultrasonography and computed tomography in the diagnoses of 28 cases of patients with malignant diseases which included 17 cases of cholangiocarcinoma and 6 cases of cancer of caput pancreatis were different from this, i. e. they were 100% and 95. 7%, respectively. For the diagnostic accuracy of the pathogenic positions of obstructive jaundice, sonography was superior to computed tomography, i. e. 92. 9% and 85. 7%, respectively. For the diagnostic accuracy of the etiology led to obstruetive jaundice, sonography was also superior to computed tomography, i. e. 89. 3% and 80. 4%, respectively. Conclusion: Based on the facts mentioned above, the authors recomendate that ultrasonography can be take as a method worthy to be first selected for the diagnosis of obstructive jaundice.
