探讨直肠内超声对直肠癌的临床价值

Clinical Significance of Intrarectal Ultrasonographic Examination for Rectal Carcinoma

目的:探讨直肠内超声检查对直肠癌术前分期的应用价值。材料与方法:直肠癌患者50例,男26例,女24例,年龄25-36岁。采用Aloka-520型实时超声仪进行直肠内检查。结果以改良Dukes'分期法为标准与病理检查相比较。结果:对浸润层次的判断有46例均与病理一致,r=0.9252符合率为92％;对淋巴结转移的检测与病理结果的符合率为94％;直肠内超声分期与病理的分期相关系系数为0.9628,符合率为92％。结论:直肠内超声能准确判断病灶所在层次,对病变周围淋巴结检查更为准确,分期有助于保肛手术的术后复查及预后估计。

Purpose: To understand the applicable value of intrarectal ultrasonograhic examination for pre-operative split - phase of rectal carcinoma. Materials and Methods: All fivty patients (male 26, female 24; aged 25 - 36 years) with rectal carcinoma originally underwent intrarectal ultrasonographic examination. The instrument used in this study was Aloka type - 520 real - time ultrasonoscope. Improving Duker’s split - phase method was taken as the standard, the results obtained from ultrasonography was compared with that from postoperative pathology. Results: The identifications of infiltrated layers in forty - six patients were corresponding to pathologic results (r = 0. 9252), the corresponding rate was 92 percent. The identifications of metastasizing into lymph nodes around lesions were corresponding to pathology at 94 percent of corresponding rate. The correlation coefficient and corresponding rate between intrarectal ultrasonographic split - phase and postoperative histopathologic results were 0. 9628 and 92 percent, respectively. Conclusion: Intrarectal ultrasonography can accurately identify the layer that is infiltrated by the lesions of carcinoma, in particular, can more accurately detect whether there to be metastasis in lymph nodes around the lesions, in addition, ultrasonographic split - phase method is very helpful to perform post -operative reexamination of holding anus operation and to predict the prognosis.
