AmpCβ-内酰胺酶研究进展 被引量:7

摘要 AmpC酶是在革兰阴性杆菌中发现的一类由染色体介导的水解头孢菌素的I型 β 内酰胺酶。AmpC酶具有诱导性 ,以其诱导酶和非诱导酶的形式存在。与ESBLS不同的是AmpC酶对三代头孢耐药且不被克拉维酸所抑制。高产AmpC酶的菌株引起的感染使现代抗感染治疗面临严峻挑战。AmpC酶的表达及调控与ampC、ampR、ampD、ampG有关。AmpC不但可由染色体编码 ,而且也可由质粒介导 ,质粒介导的AmpC酶的出现增强了耐药菌株的横向传播能力。AmpC酶的检测包括头孢西丁三相试验 ,等电聚焦电泳以及PCR等方法。目前治疗由产AmpC菌株引起感染的药物十分有限 ,可供选择的药物主要有碳青霉烯类(亚胺培南 ) ,第四代头孢菌素 (头孢吡肟 ,头孢匹罗 )等。
