髂耻束的应用解剖学观测

Anatomic observation of iliopectineal tract

目的 :观测髂耻束的位置、形态及毗邻关系 ,为临床外科腹股沟疝修补术提供应用解剖学资料。方法 :对 1 9具 (38侧 )成人尸体 ,按临床手术入路逐层进行解剖 ,暴露腹横筋膜 ,用局部解剖学方法进行观察和测量。结果 :髂耻束为腹模筋膜在腹股沟韧带深面的增厚部分 ,与腹股沟韧带平行 ,参与腹股沟管后壁的组成。男性髂耻束的长度为 51 0 1± 6 44mm ,女性髂耻束的长度为 43 2 4± 3 1 4mm ,有显著性差别 (P <0 0 5)。髂耻束的宽度、厚度无性别的显著性差别 ,宽度为 6 76± 1 0 4mm ,厚度为 0 31± 0 0 8mm。结论 :髂耻束是腹横筋膜增厚部分所形成的结构 ,与其相邻的未增厚部分的腹横筋膜 (其厚度为 0 0 4± 0 0 3mm)有明显差别 (P <0 0 5) ;髂耻束与腹股沟韧带有明显区分 ;

Objective:To observe the position,morphology and abutting relation of the iliopectineal tract in order to provide anatomic data for clinical inguinal hernia repair.Methods:To make dissection to double inguinal zone of 19 adult corpses according to the clinical operation,and to observe the inguinal zone according to the method of the regional anatomy.Results:Iliopectineal tract is the thicken part of the transverse fascia under the inguinal ligament,paralleling the inguinal ligament,and it takes part in the constitution of the posterior wall of the inguinal canal.The length of the male iliopectineal tract is 51.01±6.44mm,The length of the female iliopectineal tract is 43.24±3.14mm.There is the remarkable difference between the length of the male iliopectineal tract and that of the female one.The width of the iliopectineal tract is 6.76±1.04mm,the thickness of that is 0.31±0.08mm.Conclusion:It is confirmed that iliopectineal tract is the thicken part of the transverse fascia.Iliopectineal tract is different from the normal transverse fascia nearby the iliopectineal tract remakably( P <0 05)and the inguinal ligament.There is the clinical signification of the iliopectineal tract in the Shouldice inguinal hernia repair.
