彩超引导下经皮穿刺肝肿瘤滋养动脉栓塞化疗初探

Study of percutaneous hepatic tumor feeding arteries puncture and embolitic chemotherapy for liver tumor by the guidance of color Doppler ultrasonography

目的 探讨彩超引导下经皮肝肿瘤滋养动脉栓塞化疗的可行性与临床价值。方法 在彩超引导下,对7例因各种原因不能进行肝动脉栓塞化疗的患者,行经皮肝肿瘤滋养动脉穿刺栓塞化疗。结果 7例肝肿瘤患者,共穿刺10次,5次直接穿刺命中靶血管,5次经调整针尖,在边退针,边抽吸中穿中靶血管,总成功率100％。术后CT扫描肿瘤局灶性增强4例,肿瘤并周围肝实质普遍增强3例,肿瘤缩小2例。存活1年以上的3例。结论 彩色超声引导下经皮肝肿瘤滋养动脉栓塞化疗方法简便,安全、可行,对不能进行肝动脉栓塞化疗的大肝癌患者,提供了一种新的治疗方法,值得进一步探讨研究。

Objective To explore the feasibility and the clinic value of percutaneous hepatic tumor feeding arteries puncture and embolitic chemotherapy ( PTACE) for liver tumor by the guidance of color Doppler ultrasonography( CDU) . Methods seven cases with liver tumor, that can not be performed TACE, were treated with PTACE under CDU. Results seven cases were punctured 10 times. The needles were punctured into the target vessel in 5 times directly and the others hit target by the needles drawing. The total rate of success is 100% . The tumors in 4 cases is enhancement on CT,the tumors and the liver tissue around the tumors were enhancement in 3 cases. The tumors of 2 cases were reduced. 3 cases had been survived for one year0 Conclusion The PTACE under CDU is simple,secure,feasible. It is a new therapy method for the patients with large liver tumors and for whom the TACE can't be performedo It is valuable to further study and discuss.
