原发性小肠肿瘤142例诊治分析

Analysis of diagnosis and therapy of 142 cases of primary small intestine tumor

目的 探讨小肠肿瘤的临床病理特点 ,提高对本病的诊治水平。方法 对1968年1月至2001年9月本院收治的142例小肠肿瘤的临床资料进行回顾性分析。结果 142例小肠肿瘤中 ,良性43例 ,恶性99例。良性以平滑肌瘤为多 ,占53.5 %(23/43) ;恶性以腺癌和淋巴瘤为多 ,分别占45.4%(45/99)和34.3 %(34/99)。腹痛、腹部肿块、肠梗阻及便血是小肠肿瘤的四大常见临床表现。本组术前确诊37例 ,仅占26.1%。十二指肠镜、CT和B超对十二指肠肿瘤有较高的诊断率 ,小肠造影、DSA对诊断空回肠肿瘤有一定的作用。结论 小肠肿瘤临床表现无特异性 ,术前诊断较困难 ,临床上要提高警惕。

Objective To explore the clinical pathological characters of small intestine tumor, improve the level of diagnosis and therapy. Methods Retrospective analysis of 142 clinical data of patients with small intestine tumor who were admission from January of 1968 to September of 2001. Results 43 of 142 cases of small intestine tumor were benign, the others were malignant. Most of benign tumors were leiomyoma, it was 53.5%(23/43); most of malignant tumors were adenocarcinoma and lymphoma, respectively was 45.4%(45/99) and 34.3%(34/99). The four manifestation of small intestine tumor were abdominal pain?abdominal mass?intestinal obstruction and hematochezia. In this study, only 37 cases (26.1%) were final diagnosis preoperative. Duodenoscope?CT and ultrasoundscanner have better diagnosis, opacification of small intestine and digital subtraction anglography(DSA) have some effect in diagnosis of small intestine tumor. Conclusion Manifestations of small intestine tumor have no specificity, preoperative diagnosis is difficult, so we should be on the alert.
