摘要
本文通过监测61例腹部大、中手术前后血中PFN,CRP和AT-Ⅲ水平的变化,发现感染和手术可引起PFN、AT-Ⅲ;Ag和AT-Ⅲ:C明显降低;CRP明显升高.术后无感染或术前感染经手术等治疗控制者,术后3天PFN、AT-Ⅲ:Ag:AT-Ⅲ:C迅速回升和CRP迅速下降,8~11天内恢复正常.若术后感染或术前感染未能经手术控制时,则PFN、AT-Ⅲ回升缓慢.CRP下降缓慢或出现波动.严重感染发生MOF死亡病员,PFN和AT-Ⅲ持续处于极低水平.提示手术前后监测血中PFN。
We have conducted surveillance on 61 cases for the changes of PFN, CRP and AT-III in the blood before and after abdominal operations, we found that infection and ope ration can cause obvious drop in PFN, AT-III and AT-III:C and obvious increase in C RP.If there is no post-operative infection after operation or the preoperative infection i s well-controlled by operation,PFN, AT-III: Ag and AT-III:C rise rapidly and CRP dr ops readily 3 days after operation,and they return to normal by 8 to 11 days.If postoperative infection or the preoperative infection is not controlled by operation, PFN, AT-III rise slowly,and CRP drops slowly or fluctuates.In those patients who finally died of M OF caused by severe infection,PFN and AT-III continue to be very low levels. This st-udy shows that PFN,CRP and AT-III monitoring before and after operation may help t-o early diagnose infection and predict therapeutic effectiveness and prognosis
