摘要
目的 :研究应否及如何告知患者癌症诊断。方法 :在定性研究的基础上采用自答问卷调查来自广州市两所肿瘤医院和广东省生命之光俱乐部 ,清楚本人诊断的癌症患者 311名 ,系统了解患者对于应否和如何告知癌症诊断的看法、经历和期望 ,分析影响因素。结果 :72 99%患者认为“应该告知” ,2 4 12 %认为应“因人而异” ,2 89%认为“不应告知” ;39 2 4 % 6 0岁以上患者持后两种观点 ,比例显著高于其他年龄组。大部分患者期望肿瘤科医师在最短时间内 ,面对面地 ,在医院环境内 ,以关心同情或较好接受的态度告知其或及其家属癌症诊断 ,5 3 4 5 %较高学历者希望参与治疗方案的制定。年龄、性别、职业、学历、病期、病程对其观点有一定影响 ,而个性则无明显影响。结论 :肿瘤科医师应根据患者具体情况 。
Objective: To study whether and how to disclose the diagnosis of cancer Methods: A self-report questionnaire was designed for this study based on previous research and qualitative data generated from focus groups A total of 311 cancer patients from two tumor hospitals of Guangzhou and 'Light of Life' club of Guangdong answered questions of whether and how to disclose the diagnosis of cancer as well as their experiences and preferences with communication at the time of diagnosis Comparsons were made between patient's experiences and preferences The factors which impact patient preferences were studied Results: 72 99% patients believed the patients should be told their cancer diagnosis, while 24 12% thought Whether to disclose should depend on patient concrete conditions and only 2 89% thought that patients should not be told their cancer diagnosis 39 24% patients over 60 years old held the second and third views, the proportion was significantly higher than that of the others The majority of patients preferred to be told their cancer diagnosis by onchogists in the shortest time, face to face, with caring or sympathetic or comfortable attitude in hospitals, Age, gender, occupation, highest deucation level, period and course of disease had an impact on the patient views Personalities had no impact on their views Conclusion: Doctors should provide information to cancer patients regarding their diagnosis properly according to their concrete conditions
