Castleman病16例临床病理分析

Castlemans disease: a clinicopathologic study of 16 cases

目的 :探讨Castleman病的临床病理特点。方法 :复习确诊为Castleman病的 16例临床资料 ,采用HE切片光镜观察 ,并对其中 10例进行免疫组化检测。结果 :本组男 8例、女 8例 ;年龄 8～ 6 2岁 ,平均 4 0岁。临床上 ,12例为局灶性、4例为多中心性。组织病理学分型 ,透明血管型 9例、浆细胞型 7例。结论 :Castleman病的诊断依赖于病理组织学形态 ,其特征性表现为淋巴组织增生性的病理组织学改变 ,除外多种可以出现Castleman病形态的病变后 ,并须结合其临床资料。免疫组织化学在Castleman病的诊断中无特异性标记 。

Purpose To investigate the clinicopathologic characteristics of Castlemans disease. Methods Sixteen cases of Castlemans disease were retrieved and analyzed morphologically, and immunohistochemical staining were performed on paraffin embedded tissues with a panel of antibodies in ten cases. Results There were eight males and eight females, with age ranged 8～62 years (average 40 years). Clinically, twelve cases were local and four cases were multicentric. According to histopathologic subclassification, nine cases were hyaline vascular type, and seven cases were plasma cell type. Conclusions The diagnosis of Castlemans disease depends on its histopathological characteristics. Its characteristic change is hyperplasia of the lymphoid tissue. Other disease with similar morphologic manifestations to Castlemans disease should be excluded. Its clinical manifestation should also be taken into account. Immunohistochemical staining reveals no specific markers in the diagnosis of Castlemans disease, but will be of help in the differential diagnosis.
