弹性义齿修复的临床效果观察

Observation on clinical effects of flexible denture prosthesis in 4 years

目的 :了解弹性义齿修复的临床效果 ,为临床应用提供参考。方法 :15 2例门诊缺牙患者 ,制作弹性义齿修复体 188件 ,上颌 130件 ,下颌 5 8件。Kennedy一类缺损修复 6件 ,Kennedy二类缺损修复 4件 ,Kennedy三类缺损修复 10 2件 ,Kennedy四类缺损修复 76件。通过 1～ 4年的观察 ,从形态和功能、固定和稳定、颜色和美观以及对组织保健判断对义齿的满意程度 ,并进行临床效果评价。 结果 :79例 95件弹性义齿具有良好的美观效果 ,颜色满意 ,感觉舒适 ,满意和较满意率达 10 0 % ,Kennedy三、四类缺损修复率为 94 .74 %。弹性义齿对 Kennedy一类缺损修复的满意和比较满意率为 6 6 .6 7% ;对 Kennedy二类缺损修复的满意和比较满意率为 5 0 .0 0 % ;对 Kennedy三类缺损修复的满意和比较满意率为 96 .15 % ;对 Kennedy四类缺损修复的满意和比较满意率为 84 .2 1%。 结论 :(1)弹性义齿是一种美观、舒适、对基牙预备甚少的可摘局部义齿修复形式。 (2 )

Objective: To explore the clinical effects of flexible denture prosthesis and to apply reference to clinical application. Methods: 152 patients with partial anodontia were involved and 188 flexible denture prosthesis, including 130 for maxillar and 58 for mandibular, were made for them. In the flexible denture prosthesis,6 for Kennedy Classification Ⅰ,4 for Kennedy Classification Ⅱ,102 for Kennedy Classification Ⅲ and 76 for Kennedy Classification Ⅳ. After observation of 1~4 years, we judged the satisfactory rate to assess the clinical effects in the aspects of morphology, function, retention, stability, colour, esthetics and the ability of protecting supporting tissues. Results: 95 prosthesis of 79 patients reexamined showing favorable esthetic effect. The satisfactory rate of colour and comfort were 100%. The prosthetic rate of anterior teeth and premolar teeth were 94.74%. The satisfactory rate and relative satisfactory rate were 66.67% for Kennedy Classification Ⅰ, 50% for Kennedy Classification Ⅱ, 96.15% for Kennedy Classification Ⅲ and 84.21% for Kennedy Classification Ⅳ. Conclusion: 1. Flexible denture is a kind of esthetic and comfortable partial denture with litle abutment teeth preparation. 2. Flexible denture is relative adaptable to anterior and promolar teeth with low mastication force and few number of tooth anodontia.
