腹腔镜阑尾切除术与开腹阑尾切除术的临床效果比较研究

Comparison of clinical effects between laparoscopic appendectomy and open appendectomy

目的探究腹腔镜阑尾切除术与开腹阑尾切除术的临床效果及比较各自优缺点。方法自2014年9月~2015年9月期间,通过统计并分析我院行阑尾炎切除术患者52例,其中行腹腔镜阑尾切除术31例,开腹阑尾炎切除术21例,比较两种术式的切口长度、术中出血量、手术时间、下床活动时间、术后肛门排气时间、置管引流时间、抗生素使用时间、切口感染发生率、住院时间。并对腹腔镜手术组的术中、术后并发症及其发生因素进行探讨。结果腹腔镜阑尾切除术在手术切口长度明显小于传统开腹阑尾切除术,同时腹腔镜组术中出血量明显少于开腹术组,并且手术时间、下床活动时间、术后肛门排气时间、置管引流时间、抗生素使用时间及切口感染发生率、住院时间等方面腹腔镜手术组也明显优于传统开腹术组。各组比较差异显著具有统计学意义(P<0.05)。结论腹腔镜阑尾炎切除术相比传统开腹阑尾炎切除术在治疗急性阑尾炎手术方面具有非常明显的治疗效果及重要临床意义,并且与传统去阑尾炎切除术相比,具备快速缓解病情,术后并发症的发生率较低,住院总时间缩短等优势,值得在临床上普及应用。

Objective To explore the clinical effect of laparoscopic appendectomy and open appendectomy and compare their advantages and disadvantages.Methods from September 2014 to September 2015,52 cases of patients underwent laparoscopic appendectomy,31 cases were performed laparoscopic appendectomy,21 cases were treated by laparoscopic appendectomy,two cases were compared with kinds of incision length,bleeding volume,operation time,postoperative anal exhaust time,time of drainage,antibiotics usage,incision infection,hospitalization time.And the complications and the occurrence of complications after laparoscopic operation were discussed.Results the length of laparoscopic appendectomy was significantly less than that of traditional open appendectomy,and the amount of bleeding in laparoscopic group was significantly less than that of open operation group,and the operation time,bed time,postoperative anal exhaust time,drainage time,duration of antibiotic use,hospitalization time,hospitalization time and so on were significantly better than the traditional open operation group.The difference was statistically significant(P<0.05).Conclusion laparoscopic appendectomy compared with the traditional open appendectomy in the treatment of acute appendicitis surgery has very obvious therapeutic effect and important clinical significance,and compared with the traditional appendectomy,with a rapid relief of the disease,the incidence of postoperative complications is low,the total time of hospitalization and other advantages,it is worthy of popularization in clinical application.
