桥本氏甲状腺炎与弥漫性甲状腺肿的超声特点及其中医证型分析

Analysis of Hashimoto's thyroiditis and goiter ultrasound characteristics and TCM Syndromes

目的对桥本氏甲状腺炎(HT)和弥漫性甲状腺肿(GD)的超声特点以及中医证型进行分析。方法随机选择2014年1月至2016年12月期间我院接诊的HT、GD患者各47例,另外选择正常人群50例作为对照,分别对甲状腺大小、内部回声、甲状腺上动脉PSV、RI进行测定,并对其中医证型进行分析。结果 HT与GD患者甲状腺厚度增加、内部回声弥漫性降低,腺体内网格样改变以及低回声小结节方面,HT显著差别与GD(P<0.05);HT、GD组甲状腺上动脉PSV高于对照组,GD显著高于HT(P<0.05),RI无差异;在中医证型方面,HT多表现为肝郁脾虚型,GD多为肝郁化火型。结论甲状腺内部回声、腺体内低回声小结节数目、甲状腺上动脉PSV、中医证型在诊断HT和GD的临床实践中具有重要的应用价值。

Objective To analyze the ultrasonographic features and TCM syndromes of Hashimotos thyroiditis(HT) and diffuse goiter(GD).Methods Fifty-seven patients with HT and GD admitted to our hospital from January 2014 to December 2016 were randomly selected.Fifty patients with normal thyroid gland size,internal echo,superior thyroid artery PSV and RI were selected as controls.And the TCM syndromes were analyzed.Results HT and GD patients with thyroid thickness increased,the internal echo diffuse reduced,glandular changes in the gland and hypoechoic nodules,HT significant difference(P<0.05);HT,GD group of superior thyroid artery PSV GD was significantly higher than that of the control group(P<0.05).There was no difference in Rl between the two groups.Conclusion The internal echo of the thyroid gland,the number of hypoechoic nodules in the gland.the superior thyroid artery PSV and the TCM syndromes are of great value in the clinical practice of diagnosing HT and GD.
