摘要
目的 :探讨黄芪地龙汤对原发性肾小球疾病 (PGD)患者血清NO的影响。方法 :将 6 0例PGD按肾功能分A、B两组 ,A组又随机分A1、A2 组。A1与B组用黄芪地龙汤加西医治疗 ,A2 组单用西医治疗 ;用比色法测定三组病人血清NO浓度并结合血清肌酐 (Scr)等临床指标分析。结果 :治疗前A组病人血清NO明显高于对照组 ,B组显著低于对照组 ;治疗后血清NO水平A1组显著降低 ,B组显著升高 ;各临床指标A1组与B组明显改善 ;B组治疗前后Scr与血清NO作相关分析呈负相关 (r =- 0 .6 115 ,P <0 .0 2 ,r =- 0 .6 2 91,P <0 .0 1) ,1/Scr的平均降低值 ,A2 组 >B、A1两组 ,1/Scr与病程作回归分析 ,其回归系数 (b) ,A2 <A1与B组比较 (P均 <0 .0 0 1)。结论 :黄芪地龙汤治疗PGD能促进病情恢复 ,延缓慢性肾衰竭的进展。可能通过调节血清NO而起作用。
Objective:To investigate the effect of HQDLD on serum nitric oxide(S NO )in primary glomerulodisease(PGD).Methodes:60 PGD were divided into group A(44 cases) and B(16 cases)on renal insufficiency.Group A were randomly divided into group A 1 and A 2,group A 1 and B were treated with HQDLD and western medicine and group A 2 with western medicine alone.S NO of three group was measured by Colorimetric Method and was together analysed with mean arterial pressure(MAP)?urine protein(UP)?serum creatin(Scr) and cholesterol(Sch) and protein(S A) and so on.Results:Before the treatment,the concentration of S NO was significantly higher than of control ( P <0.001)in group A and evidently lower than of that in group B( P <0.001);After the treatment,S NO was raised remarkably in group B and significantly reduced in group A 1( P <0.001),and only somewhat reduced in group B( P >0.05);And MAP?UP?Sch and BV significantly were reduced in group A 1 and B and somewhat reduced in group A 2( P >0.05),and S A was remarkably reised in group A 1 and B,and was had a slight increase in group A 2 ( P >0.05).Scr was not obviously changed in group A 1and evidently increased in group A 2 and significantly reduced in group B.Before and after the treatment,group B there was a negative correlation between Scr and S NO ( r =-0.6114, P <0.02 and r =-0.6291, P <0.01).The mean drop rate of reciprocal Scr(1/Scr) was A 2>B and A 1,the slope of 1/Scr to time was A 2<A 1 and B by retression analysis( P <0.001).Conclusion:The treatment of PGD with HQDLD not only capacitated the recovery of UP?MAP?Sch S A and BV so on ,but also possibly postponed the development of chronic renal insufficiency.It possibly was that S NO level was reduced in early stage of PGD and increased in later period of PGD.Thus glomerulosclerosis was prevented.
