标准化流程在小儿心内直视术后非计划性床边开胸中的应用

Study on the effect of unplanned bedside thoracotomy after open heart surgery in children with standardized procedure

目的探讨标准化流程在小儿心内直视手术后床边紧急开胸中的应用,观察、总结床边紧急开胸标准化流程的临床效果。方法选择2010年5月～2017年9月本科共行床边紧急开胸27例为研究对象,将2010年5月～2015年4月的15例设为对照组做回顾性分析,采用常规流程;2015年5月～2017年9月后的12例设为实验组,采用标准化流程管理,比较2组患儿抢救成功率、床边开胸准备时间、与床边开胸相关的住院费用方面的差异。结果实验组抢救成功率、床边开胸准备时间、与床边开胸相关的住院费用均优于对照组,差异具有统计学意义(P<0.05)。结论标准化流程应用于患儿心内直视术后非计划性紧急床边开胸抢救中获得满意疗效,值得推广。

Objective To explore the application of standardized procedures in bedside emergency thoracotomy after open heart surgery in children and observe and summarize the clinical effect of standardized procedures in bedside emergency thoracotomy.Methods From May 2010 to September 2017,27 children with emergent bedside thoracotomy were selected as the study objects,15 patients with routine nursing procedures from May 2010 to April 2015 were divided into control group and their clinical data were analyzed retrospectively,12 cases of standardized procedure managers from May 2015 to 2017 were set up as the experimental group,comparing the difference between two groups in the success rate of rescue,bedside thoracotomy preparation time and bedside thoracotomy related hospitalization costs.Results The success rate of rescue,the time of opening chest bedside and the hospitalization expenses of the experimental group were better than those of the control group,and the difference was statistically significant(P<0.05).Conclusion The application of standardized procedure to the treatment of unplanned emergency bedside thoracotomy in children with open heart surgery is worth popularizing.
