老年人肺部感染并发抗利尿激素分泌异常及心功能不全的观察

Observiation in inappropriale ADH syndrome with heart failure and pulmonary infection

目的 观察老年人肺部感染时对抗利尿激素分泌异常综合征 (SIADH)及心功能不全的临床意义 ,指导临床治疗方法 对 5 0例老年人肺部感染患者进行血清电解质、2 4 h尿钠及血 ADH测定。结果 其中有 13例患者伴有高渗尿 ,低血浆渗透压 ,低血钠 ,13例中有 6位患者并发心功能不全 ,8例患者确诊为 SIADH,另外有血电解质正常的 5例心功能不全患者 (此类患者不在本研究范围内 )。结论 老年人肺部感染可导致抗利尿激素 (ADH)分泌异常 ,同时易发生心功能不全 ,使病情更危重复杂 。

Objective To evaluate the clinical significance of SIADH and cardiac insufficiency in the aged patients with pulmonary infection.Methods Serum electrolytes,urinary sodium,serum ADH were detected in 50 aged patients with pulmonary infection.Results Thirteen cases had plasma hypo osmolarity,urinary sodium hyper osmonarity,hyponatiemia,6 case of them had heart failure and SIADH were diagnosed in 8 cases.Conclusion Pulmonary infection in aged patients may cause SIADH and cardiac insufficiency,which is complex and dangerous problem.To recognise the condition is important and may guide the treatment.
