Effects and safety of low molecular weight heparin on the treatment of acute cerebral infarction

摘要 目的 观察低分子肝素 (LMWHs)治疗急性脑梗死的疗效和安全性。 方法 选用 96例急性脑梗死病人 ,其中 48例用常规治疗为对照组。 48例除常规治疗外 ,加用LMWHs 0 4mlScbid× 10天为治疗组。治疗前后测定BPC、PT、APTT、TT、Fg、D D。同时观察治疗效果和出血情况。 结果 治疗组于治疗后 10天 ,2 8天神经功能缺损评分较治疗前明显下降 (P <0 0 1) ,与对照组比较差异显著 (P <0 0 5 ) ;治疗组显效率明显优于对照组 (P <0 0 5 )。治疗组用药后Fg降低 ,PT延长 ,D D增高 ,与对照组比较差异显著 (P <0 0 1或P <0 0 5 ) ,两组治疗前后的BPC、TT、APTT变化不明显 ;治疗组有 5例出现皮下淤斑。 结论 急性脑梗死早期给予LMWHs有一定的抗凝及溶栓作用 ,有助于神经功能恢复 。 Objective To investigate the effect and safety of low molecular weight heparin(LMWHs) on treatment of acute cerebral infarction. Methods 96 patients with acute cerebral infarction were divided into 2 groups randomly.The routine medicines were given in control group;0.4ml of LMWHs bid,sc for 10d were added in the treatment group besides routine therapy.The BPC,PT,APTT,TT,Fg,D-D of the patients before and after treatment were determined and the effect and bleeding were observed at the same time Results The neurological deficits scores of 10d and 28d decreased obviously(P<0.01) in the treatment group, Compared with those of pretreatment.They also decreased compared with those of the control(P<0.05).The rate of significant effectiveness in the treatment group was better than those in the control group(P<0.05).The Fg was lowered,the PT was prolonged,the D-D was increased significantly(P<0.01 or P<0.05) in the treatment group.All of them had statistical significances compared with control group(P<0.01 or P<0.05).The BPC,APTT,TT did not changed(P>0.05).Five patients had hypodermic ecchymosis in the treatment group. Conclusion LMWHs was effective and safe with its combating coagulation and dissolving the thrombus functions in the early stage of acute cerebral infarction,good for the recovery of neural function.
