小切口椎板减压髓核摘除术治疗腰椎间盘突出症

Small incision laminectomy discectomy for lumbar disc herniation

目的探讨小切口椎板减压髓核摘除术治疗腰椎间盘突出症的方法和疗效。方法我科2005年9月-2008年12月采用小切口椎板减压髓核摘除术治疗腰椎间盘突出症119例,其中L4-5 73例,L5S1 26例,L3-4 7例,L4-5和L5S1双突出11例,L3-4和L4-5双突出2例。结果术后随访3-60个月,平均27个月。优99例,良16例,可4例,优良率达97%。结论 1、小切口椎板减压髓核摘除术治疗腰椎间盘突出症,效果确切、创伤小、术后康复快;2、准确确定责任椎间盘所处间隙、大小及与神经根、硬脊膜的关系,此对小切口手术尤为重要,做到有的放矢;3、熟练的手术技巧、优良的手术器械、良好的照明是手术成功的关键。

Objective To explore small incision laminectomy discectomy for lumbar disc herniation of the method and effect.Methods Our department in September 2005-December2008,the use of small incision laminectomy discectomy for lumbar disc herniation 119patients,in which L4-5 73patients,L5S1 26patients,L3-4 7patients,L4-5and L5S1pairs of prominent in 11patients,L3-4and L4-5pairs of prominent in 2patients.Results Postoperative follow-up 3to 60months,an average of 27months.Excellent 99cases,good in 16cases,general in 4cases,good rate of 97%.Conclusions:1.small incision laminectomy discectomy for lumbar disc herniation,the effect precise,less traumatic and postoperative rehabilitation rapidly;2.The responsibility of disc space in which to accurately determine the size and with the nerve root,epidural relations,this is particularly important for small incision surgery,so that targeted;3.Skilled surgical technique,excellent surgical instruments,good lighting is key to successful operation.
