结石通联合坦索罗辛治疗PCNL(经皮肾镜碎石)术后小残石疗效分析

Analysis of Clinical Effects for Small Residual Stones of Tamsulosin combined with Jieshitong in Patients with Upper Urinary Tract Calculi Treated by PCNL

目的探讨结石通与坦索罗辛联合治疗经皮肾镜取石术(PCNL)后小残石的临床效果。方法 100例上尿路结石患者随机分为观察组(n=56)与对照组(n=44),均给予PCNL治疗,对照组辅以结石通治疗,观察组在对照组基础上加用坦索罗辛治疗,对比两组患者术后小残石排出效果。结果观察组术后2周结石排出率为96.4%(54/56),显著高于对照组的65.9%(29/44)(P<0.01),且结石排出时间显著短于对照组(P<0.01);术后观察组患者肾绞痛发生率为21.4%(12/56),显著低于对照组的45.5(20/44)(P<0.05),且观察组镇痛药物使用率为3.6%(2/56),显著低于对照组的20.5%(9/44)(P<0.01);两组患者药物副作用发生率及其他治疗方法介入率比较无统计学差异(P>0.05)。结论坦索罗辛与结石通联合用于上尿路结石患者,能够有效缓解疼痛症状,促进PCNL术后小残石的排出,副作用较小,安全可靠,值得推广。

Objective To discuss the clinical effects for small residual stones of Tamsulosin combined with Jieshitong in patients with upper urinary tract calculi treated by PCNL.Methods 100 patients with upper urinary tract calculi were randomly divided into the observation group(n=56)and the control group(n=44),all the patients were treated with PCNL,the control group were given adjuvant therapy with Jieshitong,and the observation group were treated with Tamsulosin based on the control group,then the clinical effects were contrasted between the two groups.Results Stone expulsion rate of the observation group 2weeks postoperation was 96.4%(54/56),significantly higher than 65.9%(29/44)in the control group(P<0.01),and the lithecbole time of the observation group was significantly shorter than the control group(P<0.01);Postoperative renal colic incidence and the analgesic usage rate of the observation group were 21.4%(12/56)and3.6%(2/56),significantly lower than 45.5%(20/44)and 20.5%(9/44)in the control group(P<0.05 or P<0.01);but no significantly difference in the incidence of adverse drug reactions and other treatment methods intervention rate(P>0.05).Conclusion Tamsulosin combined with Jieshitong can effectively relief pain in patients with upper urinary tract calculi,and promote small residual stone excreted after PCNL,with fewer side effects,it's safe and reliable,and worthy of promotion.
