重度颅脑损伤继发急性硬膜下血肿的临床治疗

Treatment of Severe Craniocerebral Injury Induced Subdural Hematoma

目的探讨标准大骨瓣开颅治疗重度颅脑损伤继发急性硬膜下血肿的效果。方法将在本院就诊的46例重度颅脑损伤继发急性硬膜下血肿患者随机分成对照组(23例)和观察组(23例)。对照组实施常规骨瓣开颅手术,观察组采取标准大骨瓣开颅手术,对比两组血肿量、颅内压变化及术后并发症情况。结果相对于对照组,观察组患者术后1、3及7 d的血肿量更少、颅内压更低,术后并发症发生率更少(P<0.05)。结论重度颅脑损伤继发急性硬膜下血肿采用标准大骨瓣开颅手术治疗效果显著,且安全性较高。

Objective To investigate the effect of large bone flap in the treatment of craniocerebral injury secondary to subdural hematoma. Methods Forty-six patients were randomly equally divided into two group,standard large bone flap was performed The control group underwent conventional craniotomy. The observation group was treated by standard craniotomy. The two groups of hematoma volume, intracranial pressure changes and postoperative complications were compared. Results Compared with the control group, there were less hematoma in 1, 3 and 7 days after surgery, lower intracranial pressure and less postoperative complications(P<0.05). Conclusion Severe craniocerebral injury secondary to acute subdural hematoma is effective and safe with standard large bone flap.
