凝血酶联合奥曲肽治疗肝硬化合并上消化道出血的临床分析

Clinical Analysis of Thrombin Combined with Octreotide in Treatment of Hepatocirrhosis Complicated with Upper Gastrointestinal Bleeding

目的分析凝血酶联合奥曲肽治疗肝硬化合并上消化道出血的临床效果。方法选取本院2015年9月—2016年9月收治的74例肝硬化合并上消化道出血患者作为样本,以37例为一组,将其随机分为实验组和对照组两个组别。对照组在常规治疗方法的基础上,联合奥曲肽治疗。实验组在常规治疗方法基础上,采用凝血酶联合奥曲肽治疗。观察两组患者住院和治疗情况、症状改善时间、不良反应发生情况、治疗效果。结果实验组患者住院时间(6.9±5.0)d、输血量(231.6±50.1)mL、止血时间(20.6±3.7)h、症状改善时间(1.91±0.02)d、不良反应总发生率2.70%,治疗总有效率97.3%。与对照组相比,优势显著。结论应将凝血酶联合奥曲肽用于肝硬化合并上消化道出血的临床治疗。

Objective the study was designed to analyze the clinical effect of thrombin combined with octreotide in treatment of hepatocirrhosis complicated with upper gastrointestinal bleeding.Methods 74 cases of liver cirrhosis combined with upper gastrointestinal bleeding patients receiving treatment in our hospital from September,2015 to September,2016 were selected as the sample,and they were randomly divided into two groups--experimental group and control group with 37 cases in each.The control group was applied conventional treatment methods combined with octreotide treatment;the experimental group, based on the conventional treatment methods,was applied thrombin combined with octreotide treatment. Observation of the two groups was made on patients' conditions while being hospitalized and treated, duration of symptom improvement,adverse reactions,and treatment effect.Results the experimental group, with patients' hospitalization time of(6.9 + 5) d,blood transfusion volume(231.6 + 50.1) ml, time of hemostasis(20.6 + 3.7) h,duration of symptom improvement(1.91 + 0.02) d,rate of adverse reaction probability 2.70%,the total efficiency of treatment 97.3%,showed significant advantage comparing with the control group.Conclusion thrombin combined with octreotide should be utilized in clinical treatment of hepatocirrhosis complicated with upper gastrointestinal bleeding.
