输尿管镜联合气压弹道碎石治疗复杂性肝胆管结石的临床经验

Experience of Ureteroscopy Combined with Pneumatic Lithotripsy in Treatment of Complex Hepatolithiasis

目的探讨输尿管硬镜配合气压弹道碎石技术治疗肝内外胆管结石的可行性及临床价值。方法选取本院收治的28例术中采用输尿管镜气压弹道碎石治疗肝内外胆管结石患者的资料作为研究对象。结果治愈率为89.2%,残石率为10.7%;平均术中出血量为120 m L,平均手术为150 min;术后无胆道狭窄、穿孔、出血等并发症出现。结论采用输尿管镜气压弹道碎石治疗肝胆管结石具有操作简单、取石快、并发症少,可提高结石取出率,降低残石率和再手术率。

Objective To explore the feasibility and clinical value of rigid ureteroscopy combined with pneumatic lithotripsy in the treatment of extrahepatic and extrahepatic bile duct stones.Methods The data of 28 patients with intrahepatic and extrahepatic bile duct stones treated with ureteroscopic pneumatic lithotripsy were analyzed retrospectively.Results The cure rate of the group was 89.2%,the residual stone rate was 10.7%,the mean intraoperative bleeding amount was 120 m L,the average operation was 150 min,and there were no complications such as biliary stricture,perforation and bleeding after operation.Conclusion The treatment of hepatolithiasis with ureteroscope pneumatic lithotripsy has the advantages of simple operation,quick stone extraction and few complications,which can increase the stone removal rate,decrease the residual stone rate and re-operation rate.
