家庭医生签约式服务模式对社区老年患者慢性病防治的效果

Effect of Family Doctor Contract Service Mode on Prevention and Treatment of Chronic Disease of Elderly Patients in Community

目的探索家庭医生签约式服务模式在社区老年患者慢性病防治当中的效果。方法选取2016年12月至2017年12月社区老年人慢性病患者100例作为研究对象,随机分为观察组与对照组,各50例,其中对照组利用常规服务模式,观察组利用家庭医生签约式服务模式。结果观察组的健康知识知晓程度、自我控制、依从性评分均高于对照组(P<0.05);观察组的并发症发生率明显低于对照组(P<0.05)。结论社区老年患者慢性病防治过程中应用家庭医生签约式服务模式效果显著,应用价值比较高。

Objective To explore the effect of family doctor signed service mode in the prevention and treatment of chronic disease of the elderly in the community. Methods In December 2016 to 2017,100 elderly patients with chronic diseases were randomly divided into the observation group and the control group,50 cases in each group. The control group used routine service mode,and the observation group used family doctor contract service mode. Results The satisfaction rate of the observation group was significantly higher than that of the control group(P<0.05). Conclusion The application of family doctor contract service model has remarkable effect in the prevention and control of chronic disease of the elderly in the community.
