标准大骨瓣减压术与常规骨瓣开颅术治疗重型颅脑损伤的效果比较

摘要 目的对比标准大骨瓣减压术与常规骨瓣开颅术治疗重型颅脑损伤的效果。方法选取本院2016年2月至2018年3月收治的136例重型颅脑损伤患者,随机分为常规组和标准组,各68例,分别行常规骨瓣开颅术和标准大骨瓣减压术治疗,比较两组疗效。结果常规组和标准组重型颅脑损伤患者预后良好率分别为60.29%、79.41%,组间比较差异有统计学意义(P <0.05);经过手术治疗后,两组患者GCS评分、NIHSS评分均有明显改善,标准组患者GCS评分为(9.63±0.74)分,高于常规重型颅脑损伤患者,NIHSS评分为(11.15±3.85)分,低于常规重型颅脑损伤患者(P <0.05)。结论与常规骨瓣开颅术相比,标准大骨瓣减压术治疗重型颅脑损伤的效果更佳,能有效改善患者神经功能,改善患者预后。 Objective To compare the effect of standard large bone flap decompression and conventional craniotomy for severe craniocerebral injury. Methods A total of 136 patients with severe head injury who were admitted to our hospital from February 2016 to March 2018 were enrolled in the study. The lottery was used to divided all patients into the conventional group and the standard group 68 cases each. Valve craniotomy and standard large bone flap decompression were used to compare the efficacy of the two groups. Results The prognosis of patients with severe craniocerebral injury in the conventional and standard groups was 60.29% and 79.41%,respectively,and the difference was significant(P<0.05). After surgery,the GCS score and NIHSS score of the two groups were significantly improved. The GCS score of the patients was 9.63±0.74,which was higher than that of the patients with conventional severe head injury. The NIHSS score was 11.15±3.85,which was lower than that of patients with severe head injury(P<0.05). Conclusion Compared with conventional craniotomy,standard large bone decompression is better for severe craniocerebral injury,which can effectively improve neurological function and improve patient prognosis.
