膝关节前交叉韧带损伤的磁共振成像检查及临床诊断意义探究

MRI Examination and Clinical Diagnosis of Anterior Cruciate Ligament Injury of Knee Joint

目的分析膝关节前交叉韧带损伤的磁共振成像(MRI)检查及临床诊断意义。方法选取2016年7月至2018年2月本院收治的76例膝关节前交叉韧带损伤患者为研究对象。所有患者均经关节镜检查确诊,并给予MRI检查,评估患者的前交叉韧带外形、内部信号等状况。结果以关节镜检查为金标准,MRI对膝关节前交叉韧带损伤的诊断符合率为96.05%。MRI诊断共检出73例,其中,前交叉韧带中段损伤13例(17.81%),下段损伤25例(32.25%),中上段损伤35例(47.95%)。前交叉韧带部分撕裂患者的MRI征象以韧带增粗为主,前交叉韧带完全撕裂患者的MRI征象为:前交叉韧带增粗、肿胀、断裂,韧带边界不清,PDW-SPAIR可见韧带断端信号增高,T2WI显示前交叉韧带伴假团块及韧带回缩征象。结论 MRI诊断对膝关节前交叉韧带损伤的诊断符合率较高,可清晰显示患者的前交叉韧带损伤部位、形态及严重程度,值得临床推广应用。

Objective To analyze the MRI examination and clinical diagnosis of anterior cruciate ligament injury of knee joint.Methods A total of 76 cases of anterior cruciate ligament injury of knee joint admitted to our hospital from July 2016 to February 2018 were selected as the research objects.All patients were confirmed by arthroscopy.MRI examination was performed to evaluate the appearance and internal signal of anterior cruciate ligament.Results With arthroscopy as the gold standard,the diagnostic coincidence rate of MRI for anterior cruciate ligament injury of knee joint was 96.05%.A total of 73 cases were detected by MRI diagnosis,including13 cases(17.81%)of middle anterior cruciate ligament injury,25 cases(32.25%)of lower anterior cruciate ligament injury,and 35 cases(47.95%)of upper anterior cruciate ligament injury.MRI signs of patients with partial anterior cruciate ligament tear were mainly ligament thickening,MRI signs of patients with complete anterior cruciate ligament tear were anterior cruciate ligament thickening,swelling,fracture,unclear ligament boundary,increased signal at the broken end of the ligament visible in pdw-spair,T2 WI showed anterior cruciate ligament accompanied by pseudo-masses and ligament retraction signs.Conclusion MRI diagnosis has a high diagnostic coincidence rate for anterior cruciate ligament injury of knee joint,which can clearly show the site,morphology and severity of anterior cruciate ligament injury of patients and is worthy of clinical application.
