新生儿开塞露灌肠灌肠管插入不同深度的效果观察

Effect of Caiselw Enema tube Insertion in Neonates at Different Depths

目的探讨新生儿开塞露不保留灌肠灌肠管插入不同深度的临床效果。方法选取2017年5月至2018年4月扬州大学附属医院NICU收治的100例胎龄≥35周且体重≥2 051 g患儿随机分为观察组和对照组,各50例。分别进行开塞露灌肠,对照组灌肠管插入肛门3.5～4 cm,观察组灌肠管插入肛门8.5～9 cm,1次/d,连续5 d。灌肠后观察患儿初次排便时间、每次排便量、日排便次数、胎便排净时间、经皮胆红素值及日喂奶总量。结果观察组胎便排净时间低于对照组,观察组平均每次排便量、平均日排便总量及平均日排便次数均高于对照组,观察组2、3、4 d的经皮胆红素值低于对照组,观察组日均喂奶总量高于对照组,观察组接受光疗率低于对照组(P <0.05)。结论新生儿开塞露灌肠时增加插入深度能帮助患儿尽早排净胎便,减轻黄疸水平,同时可加快胃排空,增进食欲,改善肠胃功能。

Objective To investigate the clinical effect of neonatal sputum dew on the different depths of enema and enema.Methods From May 2017 to April 2018,100 cases of infants with gestational age 35 weeks and body weight 2 051 g admitted to NICU affiliated hospital of yangzhou university were randomly divided into observation group and control group,50 cases each.respectively,Kaisailu enema,control group enema insertion into the anus3.5~4 cm,the observation group enema tube inserted into the anus 8.5~9 cm,once a day for 5 consecutive days.After enema,the initial defecation time,the amount of each bowel movement,the number of daily bowel movements,the net time of fecal discharge,the percutaneous bilirubin value and the total daily feeding amount were observed.Results Observation group net meconium discharge time is lower than the control group,the observation group average each defecation quantity,average daily total defecate and average daily stool frequency are higher than the control group,the observation group 2,3,4 d transcutaneous bilirubin value is lower than the control group,observation group,daily feeding amount is higher than the control group and observation group accepted phototherapy rate is lower than the control group(P<0.05).Conclusion Increasing the depth of insertion of neonatal sputum enema can help the child to remove the fetus as soon as possible,reduce the level of jaundice,accelerate gastric emptying,increase appetite and improve gastrointestinal function.
