超声乳化对角膜内皮细胞影响的研究

Effects of phacoernulsification on the corneal endothelium

目的:了解白内障超声乳化摘除术对角膜内皮细胞的早期影响。方法:比较白内障超声乳化摘除术36眼和白内障囊外摘除术32眼术前及术后第二天角膜内皮细胞的变化。结果:角膜内皮细胞丢失超声乳化组为491±208个/MM^2,占18.3±7.5%;囊外组为420±225个/MM^2占17.9±7.2%。两组比较差异无显著性(t检验,P>0.05)。结论:在白内障超声乳化摘除术中应尽量减少超声能量和超声时间,超声能量小于30%,时间小于5分钟对于角膜内皮细胞是安全可靠的。

To investigate early postoperative corneal endothelial damage caused by phacoemulsification. Methods; Cell density of the corneal endothelium was examined preoperatively and 2 days postoperatively in 36 eyes which received phacoemulsification (Phaco group) and in 32 eyes which received extracapsular cataract extraction (ECCE group). Results; Mean cell loss of corneal endothelium was 491±208/mm2 in thePhaco group and 420 ± 225/mm2 in the ECCE group; Percentage of endothelium cell loss was 18.3 ±7.5% in the Phaco group and 17.9 ±7.2% in the ECCE group. There was no statistically significant difference between the two groups (P> 0.05, Primer t test). Conclusion: Our study suggests that time and power of phacoemulsification should be reduced as much as possible. It is safer for corneal endothelium when phaco time is less than 5 minutes and power is lower than 30%.
