无眩晕反应改良人工镫骨手术

No Postoperative Vertigo in Partial Stapedectotny with Modified Stapedial Prosthesis

目的设计一种新型人工镫骨,防止镫骨部分切除术后常见的眩晕反应。方法在小窗技术镫骨部分切除术中,将人工镫骨足端斜截45°,插深0.5mm。结果 133耳耳硬化症用新型人工镫骨,92耳手术(85.2%)无眩晕出现。余例眩晕甚轻、时间短。在常规人工镫骨手术中出现术后眩晕有163耳(93.7%)。结论用新型人工镫骨代替常规人工镫骨可很大程度地预防镫骨部分切除术的术后眩晕。

PURPOSE To design a new type of Stapedial prosthesis for prevention of postoperative vertigo, which often occured in partial stapedectomy with regularprosthesis.METHOD The terminal of prosthesis intercepted with a slope of 45° is positioned in the vestibule maintained at 0.5mm depth,whenever the partial stapedectomy fenestration technique is ready.RESULTS In 108 surgeries in otosclerosis with new stapedial prosthesis, no postoperative vertigo presented in 92 (85.2%) with only slight and short-term vertigo in remained cases. Postoperative vertigo occured in 163 surgeries (93%) in 174 ears with regular prosthesis.CONCLUSION To substitute new stapedial prosthesis for regular one can prevent the patient from postoperative vertigo to a great extent.
