摘要
目的探讨CD44及PCNA在喉癌组织中的表达及其与喉癌组织分化、增殖、浸润、转移,以及生存率之间的相关性,研究它们在喉癌治疗、预后评价中的应用意义。方法应用免疫组织化学方法(ABC法)对45例喉癌组织CD44及PCNA表达进行检测。结果 CD44及PCNA表达的分布具有一致性,高分化鳞癌CD44及PCNA表达明显低于低分化鳞癌(P<0.005,P<0.01);伴颈淋巴结转移的喉癌组织CD44及PCNA表达明显高于无颈淋巴结转移组(P<0.001,P<0.01);临床T_3、T_4喉癌CD44表达明显高于T_1、T_2(P<0.001)。CD44及PCNA高表达的喉癌患者3年生存率明显低于CD44及PCNA低表达者(P<0.001,P<0.01)。结论 CD44表达与喉癌组织的分化、增殖、进展及转移密切相关,CD44高表达的喉癌组织侵袭力强,增殖活跃,恶性程度高,易发生颈淋巴结转移,临床上应予以高度重视。
PURPOSE To explore expressions of CD44 and PCNA in laryngeal carcinoma tissues and their correlations with differentiation,proliferation,invasion,metastasis of laryngeal carcinoma as well as survival rate and to evaluate the significancies of their applying to laryngeal carcinoma therapies and prognoses.METHODS Immunohistochemical assessment(ABC method)was applied to detect expressions of CD44 and PCNA in 45 cases of laryngeal carcinoma.RESULTS The distribution of CD44 expression conformed to that of PCNA expression; expressions of CD44 and PCNA in highly differential squamous carcinoma significantly lower than those in lowly differential squamous carcinoma(P<0.005,P<0.01 respectively); expressions of CD44 and PCNA in laryngeal carcinoma tissues with cervical lymph node metastasis were markedly higher than those of no lymph node metastasis group(P<0.001,P<0.01 respectively); the expression of CD44 in clinical T3,T4 stages laryngeal carcinoma was obviously higher than that in T1,T2 stages(P<0.001).3 year survival rate in laryngeal carcinoma patients with high expression of CD44 and PCNA was markedly lower than those patients with low expressions of these two indi-ces(P<0.001,P<0.01 respectively).CONCLUSIONS CD44 expression was highly correlated with differentiation,proliferation,development and metastasis of laryngeal carcinoma; laryngeal carcinoma tissues with high CD44 expression were characterized by potent invasive capability,highly proliferative activities,high malignant degree and prone to metastasize to cervical lymph nodes,which should be paid more attention.
