摘要
目的探讨喉癌变过程中CyclinE、CyclinD1和P21^(WAFI/CIP1)表达的临床病理学意义。方法用免疫组化检测20例正常粘膜、40例不典型增生病变和60例喉癌组织中CyclinE、CyclinD1和P21^(WAFI/CIP1)的表达。结果①CyClinE、CyClinD1和P21^(WAFI/CIP1)阳性表达定位于细胞核。②在喉癌变过程中,喉正常粘膜、不典型增生病变和喉癌中CyclinE阳性表达率分别为:5.0%(1/20),20.0%(8/40),45.0%(27/60)(P<0.01);CyclinD1阳性表达率分别为:5.0%(1/20),30.0%(12/40),53.3%(32/60)(P<0.001);P21^(WAFI/CIP1)阳性表达率分别为:95.0%(19/20),75.0%(30/40)和63.3%(38/60)(p<0.05)。③P21^(WAFI/CIP1)在高、中和低分化的喉癌中阳性表达率分别为:76.2%(16/21),65.5%(19/29)和30.0%(3/10)(P<0.05)。④CyClinD1和P21^(WAFI/CIP1)阳性表达显著相关,CyClinE与Cy-clinD1,CyclinE与P21^(WAFI/CIP1)表达无相关性。结论①喉癌变过程中CyclinE和CyclinD1阳性表达率呈逐渐升高的趋势,而P21^(WAFI/CIP1)阳性表达率呈逐渐降低的趋势。②CyclinE和CyclinD1异常表达是喉癌发生中早期分子事件。③P21^(WAFI/CIP1)表达与喉癌细胞分化程度有关。④细胞周期调控因子CyclinE、CyclinD1和P21^(WAFI/CIP1)相互调控机制在喉癌发展中起重要作用。
PURPOSE To investigate the clinicopathological significance of the Cycling CyclinDl and P21WAFI/CIPI expression in laryngeal carcinogenesis. METHODS The expression of CyclinE, CyclinDl and p21WAFI/CIPI was examined by immunohistochemistry method in normal epithelia (20) dysplastic epithelia(40) and laryngeal cancers (60) .RESULTS ① Immunoreaction to CyclinE , Cy-clinDl and P21WAFI/CIPI was localized in the nuclei of cells.②In normal epithelium, dysplastic squamous epithelium and in squamous cell carcinoma of the larynx, the positive immunostaining rate of CyclinE protein was 5. 0% (1/20) , 20. 0% (8/40) and 45. 0% (27/60) respectively (P < 0.001), CyclinDl protein was 5.0% (1/20), 30.0% (12/40)and 53.3%(32/60) respectively (P < 0. 001) and P21WAFI/CIPI protein was 95.0%(19/20) ,75.0% (30/40) and 63.3 % (38/60) respectively ( P < 0.05). ③ P21 positive expression in highly, moderatelyand poorly differentiated carcinomas were 76. 2 % (16/ 21),66.5%(19/29) and 30.0%(3/10) respectively (P < 0.05). ④ The positive expression of CyclinDl was closely related with the positive expression of p21WAFI/CIPI ( P < 0. 05). No correlation was found between CyclinE and CyclinD1 and p21WAFI/CIPI (P > 0.05).CONCLUSION ① With the development of lesions the positive immunostaining rate of CyclinE and CyclinDl protein gradually increases, while the rate of P21 WAFI/CIPI protein gradually decreases. ② CyclinE andCyclinD1 can be the early molecular events in the carci-nogenesis of larynx.③ P21WAFI/CIPI expression is associated with tumour cell differentiation. ④ The mutual mechanism of cell cycle regulators plays an important role in the laryngeal carcinoma.
