米索前列醇预防产后出血的临床研究

Clinical study for Misoprostol to prevent and cure the postpartum bleeding

目的:寻找一种简便、安全、高效的防治产后出血的新药方。方法:选择在该院分娩无妊娠合并症、自然临产、无米索前列醇应用禁忌症的产妇300例,随机分成米索前列醇组(观察组)与催产素组(对照组)进行对比观察。结果:观察组比对照组第三产程明显缩短,产后2 h出血量观察组为135.2 ml,对照组为163.4 ml,产后24 h出血量,观察组为297 ml,对照组为352 ml。观察组产后2 h、24 h出血量明显低于对照组。统计1 139例分娩产妇,发生产后出血34例,发生率2.99％。在研究范围内产后出血共12例,催产素组9例,产后2~24 h内出血4例,占44.44％,米索前列醇组3例,均发生在产后2 h内。结论:米索前列醇直肠给药安全、有效,副作用少,优于催产素,是防治产后出血值得首选的药物。

Objective: To seek a new medicine used to prevent and cure the postpartum bleeding simply and safely and effectively. Methods: 300 pregnant women in our hospital were selected in study group. All of them were normal spontaneous deliveries, without reaction complication of pregnancy and without application contraindication of Misoprostol. They were randomized into 2 groups for contrast observation: misoprostol group (the observation group) and oxytocin group (the reference/ comparison group). Results: The third stage of labor in observation group was significantly reduced. The amount of bleeding in 2 hours after delivery was measured. The results: the amount of bleeding in observation group was 135. 2 ml and it was 163. 4 ml in the reference group; while in 24 hours, the amount of bleeding in observation group was 297 ml and it was 352 ml in the reference group. Whether in 2 hours or in 24 hours, the amount of bleeding in observation group was significantly lower than the one in the reference group. Counted 1139 delivery women, statistics showed that there were 34 cases of postpartum bleeding, the percentage was 2. 99%. In this study, there were 12 cases of postpartum, there - into, 9 cases in oxytocin group, 4 of them had internal hemorrhage within 2 to 24 hours, the percentage was 44. 44%; while in the misoprostol group, there were only 3 cases in postpartum bleeding, and none of them had internal hemorrhage within 2 to 24 hours. Conclusion: Misoprostol per rectum is more effective in prevention and cure of postpartum bleeding than that of oxytocin. It is a safe, efficient, less side -effects medicine that we should pick and choose.
